Agreement

By becoming a Community Partner, your organization agrees to the terms below.

01

Purpose & Collaborative Scope

This Community Partnership Agreement establishes a cooperative framework between Forward Interventions and the Partner organization. Forward Interventions provides spiritual, mental, and behavioral wellness resources designed to support the individuals and communities served by partner organizations. Your selected partnership level determines the resources, access, sessions, and other benefits available to your organization.

02

Access to Resources

Your organization will receive access to the Forward Interventions resources included with your selected partnership level. Access is intended for the individuals served by your organization and is subject to the usage limits associated with your partnership level. Forward Interventions will be adding new educational materials, videos, sessions, and other wellness resources throughout the year to qualifying tiers. Specific resources and programming may change as Forward Interventions develops and improves its wellness platform. Depending on the tier you select, resources may include:

Melodies of Victory Victorious Overcomer Forward to Victory The Victory Circle Morning Grounding Virtual wellness sessions In-person workshops or facilitation
03

Unique Access Codes

Your organization will receive a unique access code for distributing eligible resources to your community. Access codes may not be: Sold. Publicly posted. Or shared outside your organization or intended community.

04

Virtual & In-Person Sessions

Where included in your selected partnership level, Forward Interventions will provide the virtual sessions, workshops, or in-person facilitation for selected tiers. In-person services are subject to scheduling, availability, location, and reasonable travel limitations. Forward Interventions may charge additional fees such as travel & hotel for in-person sessions considerably far from Milwaukee, WI. Such details will be discussed further during the call to book such services.

05

Payments & Cancellations

By enrolling, your organization agrees to the initial payment amount and billing frequency selected during checkout. Monthly subscriptions will automatically renew each month until canceled. Annual subscriptions are paid in advance for the selected annual term. Annual partnerships remain active through the end of the purchased annual term unless otherwise agreed to in writing. Any applicable startup, enrollment, or other fees will be displayed before payment is submitted. Monthly partnerships may be canceled before the next billing cycle. Cancellation stops future recurring charges but does not create a refund for payments already processed. If a partner cancels their plan and desires to re-enter a partership with Forward Interventions, they will be subject to the prices and fees displayed on the site at the time of re-entry. Meaning, partners may lose their discounted monthly and annual tier prices if they chose to exit partnership. They will also be subject to the standard start-up fee upon re-entry depending on their selected tier.
